Please enclose this Memorial Exhibition Materials Deed of Gift form with any items or images you elect to send us. This form provides us with the information necessary to preserve your materials in our permanent archive and to include them in exhibitions, publications and educational programs. You will have the option of selecting whether or not you are comfortable with extending use of your materials to commercial endeavors the Museum might develop, typically taking the form of books, cards, and commemorative products. Once we have received and reviewed the materials, we will send you a signed copy of this form, for your records, and include any materials that you have requested to be returned.   • For information about how to best ship your materials please see our shipping guidelines.
  • Please note that we are hoping to obtain at least one clear, visually crisp portrait of each individual. It is ideal if the image shows your loved one looking at the camera, from the chest up, and was taken around the year 2000 or 2001. Kindly provide us with a photograph that is as close to the original print as possible, to insure its legibility when produced for exhibition.
  • You are welcome to send us supporting photographs along with the portrait (3-5 recommended). Donations exceeding this number will be reviewed, and a selection will be made by the Museum. The remaining images will be returned to the donor.
  • Please follow the instructions on the form for describing the materials you enclose, remembering to include your contact information and preferred credit line for the gift. Please select either “option 1” or “option 2” to grant the Museum use permissions for the materials you enclose. Please also consider granting the Museum permission to share digital copies of your materials with our partner, Voices of September 11th, for inclusion in the 9/11 Living Memorial project.
